NHS Newcastle Gateshead CCG is considering ways to improve care and support of people with one or more long term conditions. These are health problems that cannot currently be cured but are controlled by medication or treatment, for example diabetes, breathing problems, heart conditions, stroke or rheumatoid arthritis. This consultation seeks to find out what would help people living in Newcastle and Gateshead manage their conditions with confidence.
